Once your son/daughter has been offered a spot on the team you will need to fill out the information on the appropriate payment form click here for the ACH Payment Form, click here for the Credit Card Payment Form, click here for Check/Cash Payment Form. You can fax it to the number on the bottom of the form, mail it in or even drop it off at the office. The first payment will be drafted May 15th for U11 teams and on June 15th for U12-U18 teams.
